Before lifting, the nurse should assess the weight to be lifted and what assistance, if any, is needed. If help is needed, the nurse should assess if a second person is adequate or if mechanical assistance is needed. Once the amount of needed assistance id determined, these steps are followed.
1. Tighten stomach muscles and tuck pelvis; this provides balance and protects the back.
2. Bend at the knees; helps to maintain the nurse’s center of gravity and lets the strong muscles of the legs do the lifting.
3. Keep the weight to be lifted as close to the body as possible; this places the weight in the same plane as the lifter and close to the center of gravity for balance.
4. Maintain the trunk erect and the knees bent so that multiple muscle groups work together in a synchronized manner.
5. Avoid twisting. Twisting can overload your spine and lead to serious injury.
To reach an object overhead the nurse should do the following:
1. Use a safe, stable step stool or ladder for elevation. Avoid standing on tiptoes with the feet together. This decrease the base of support, elevates the center of gravity, and decreases balance.
2. Stand as close to the shelf as possible. This decreases the amount of time the nurse must support the weight of the object with the arms.
3. Transfer the weight of the object from the shelf to the arms and over the base of support. This maintains the nurse’s base of support and aligns the weight of the object close to the nurse’s center of gravity.
Moving the Patient with Assistance
Sunday, December 5, 2010
Material Needed: Drawsheet folded to use as lift sheet
EXPECTED BEHAVIOR
1. Remove the pillow and place it at the head of the bed.
2. Positioning with two nurses or staff members.
• First position : Position one nurse on each side of the patient. Each nurse should have one arm under the patient’s thighs.
• Alternative Position: Position one nurse at the patient’s upper body. The nurse’s arm nearest the head of the bed should be under the patient’s head and opposite shoulder. The other arm should be under the patient’s closest arm and shoulder. Position should be under the patient’s lower back and thighs.
• Alternative Position: Place folded draw/turn sheet under the patient’s body extending from shoulder line to just below buttocks. Position one nurse on each sides of lift sheet as close as possible to sides of patient. Assist patient to flex knees, if possible. Each nurse firmly grasps sheet at level of patient’s upper back one hand and at level of buttocks with other hand. Then with one form, coordinated, rocking movement, lift patient toward head of bed.
3. Positioning with three staff members
• Position two nurses so that each one is supporting the patient’s shoulders as described above.
• Position the third nurse at the patient’s lower torso.
4. Positioning with four staff members
• Position two nurse, one each on each side of the patient’s hips and legs.
5. Coordinate the movements of all nurse. One nurse is responsible for stating when to move patient.
6. Place patient in a comfortable position.

USING A FOOTBOARD
MATERIALS NEEDED: Footboard
EXPECTED BEHAVIOR
1. Assess patient’s ability to place feet in dorsal flexion. If unable to do so, or plantar flexion is continuous, provide a footboard.
2. Cover footboard with a bath blanket to protect feet from rough surfaces.
3. Place footboard in a place where patient’s feet can firmly rest on it without sliding down in bed.
4. Observe legs to ensure that they are not in flexed position when feet are against the board.
5. Tuck to linen under mattress at foot of bed, and bring linen up over the footboards as it can easily be pulled off the bed.
6. Put feet and ankles through range-of-motion exercises every four hours on patient with prolonged bed rests.
7. Observe heels and ankles frequently for signs of breakdown.

PRINCIPLES OF BODY MECHANICS
Monday, November 29, 2010
• The wider the base of support, the greater the stability of the nurse.
• The lower the center of gravity, the greater the stability of the nurse.
• The equilibrium of an objects is maintained as long as the line of gravity passes through its base of support.
• Facing the direction of movement prevents abnormal twisting of the spine.
• Dividing balanced activity between arms and legs reduces the risk of back injury.
• Leverage, rolling, turning, or pivoting requires less work than lifting.
• When friction is reduced between an object to be moved and the surface on which it is moved, less force is required to move it.
• Reducing the force of work reduces the risk of injury.
• Maintaining good body mechanics reduces fatigue of the muscle groups.
• Alternating periods of rest and activity helps to reduce fatigue.
• The lower the center of gravity, the greater the stability of the nurse.

Bed Making
Sunday, November 21, 2010
The health care provider makes a client’s bed with safely and comfort in mind. The sheets must always be clean, dry, and wrinkle free. Whenever a client’s bed in soiled, it must be changed. The sheets should be straightened out and tightened periodically throughout the day to keep them wrinkle free. When clients are not allowed out of bed, the nurse must make an occupied bed. Whenever a client can get out of bed, an unoccupied bed should be made and left open with the top sheets folded down. When a client is discharged and housekeeping cleans the unit, the bed is made with the top sheets left up. This is known as closed bed. For client who have gone to the operating room or a procedural area, the nurse fanfolds the top sheet lengthwise without tucking them in to facilitate the client’s return to bed. This is called a postoperative or surgical bed.
UNOCCUPIED BED
Special Consideration:
1. Before starting the procedure see to it that all linen are on hand and arranged according to use.
2. If the bed is of the Hi-Low Type, adjust the height accordingly.
3. Finish one side of the bed at a time.
4. Remember that placement of the rubber sheet will depend upon the client need.
5. Leave the client’s until in order.
6. Apply the principles of body mechanics while you are making the bed.
Equipment: Assemble of the bedside and arranges according to use the following:
2 Bed sheets 1 Bedspread (optional)
1 Rubber sheet 1 Pillowcase
1 Draw sheet 1 Mattress cover (if necessary)
OCCUPIED BED
Definition: a bed that is made with the patient on it.
Purpose:
a. To change wet or soiled beddings for the safety and comfort of the patient.
b. To provide a change of position and to afford an oppurtuinity to inspect the client’s body.
Equipment: arrange at the bedside and in order of use the ff:
2 Bed sheets 1 Rubber sheet (if it needs changing)
1 Draw sheet 1 or more pillow cases

HAND WASHING AND ASEPSIS
Thursday, October 28, 2010
Asepsis is commonly defined as the absence of pathogenic organisms. Hand washing is a proven method of asepsis, which effectively minimizes the onset and spread of infection. It is the brief but vigorous rubbing together of all the hands’ surface lathered with soap or antibacterial hand washing agents, followed by a thorough rinsing in a continuous stream of water.
The purpose of this procedure is to remove dirt and any transient organisms from the hands and the reduction of microorganism count.
Hand contamination is the most common cause of cross infection; and disease can be easily communicated from one person to another through hand contact or touching objects which have been contaminated. If a nurse fails to wash hands after handling articles, which are contaminated, he/she is most likely to spread infections.
Factors influencing Frequency of Hand Washing
• Intensity of contact with patients
• Patient/nurse’s susceptibility to infection
• Contamination degree that could result from contact
• Procedure/activity to be performed
Situations where Hand Washing is necessary
• When hands are visibly soiled
• Before and after contact with patient
• Before performing invasive procedures (e.g., intravascular catheter/indwelling catheter insertion)
• After contact with a source of microorganism (e.g., blood/bloody fluids, mucous membrane or potentially contaminated objects)
• After removal of gloves

Product Description:
Surgical Management of Spinal Cord Injury: Controversies and Consensus reviews the controversies pertaining to the emergency, diagnostic, medical, and surgical management of spinal cord injury (SCI).
In vitro studies, animal models, and clinical outcome analyses have all failed to yield incontrovertible guidelines that define the role of surgery in SCI. As a result, there is no consensus regarding the necessity, timing, nature, or approach of surgical intervention.
